I'm a 41 yr. old single male and can be very shy.  I've been lucky to have a considerable amount of good, enriching relationships, despite myself.  I am very dense.  For the past year and a half, I have attended this gym and for the first few months, this woman flirted with me and we played what could best be described as peek-a-boo.  We've never spoken and despite opportunities, I've never initiated a conversation.  So, she's given me signs of exasperation and probably some other things. Despite shyness, I was new in town, had a stressful job that preoccupied me, and the status of the relationship I was in was murky.

Does it even make sense to approach her after all of this time?

Always! Pursue her before the window closes completely. Explain things to her later if needed. But don't hold out. She may think you are just playing mindgames.
