I think it refers to the fact that the head and the heart often don't agree with each other -- but a great many people allow themselves to be led by their hearts instead of their heads much of the time even though the emotions are frequently illogical and transitory. For many people, keeping the head in charge of the heart and choosing to act instead of simply reacting seems to require a certain degree of willpower and effort -- especially when reason comes into conflict with the wishes of the ego.
