In a diplomatic manner ,yes perhaps, because I would expect somebody to do the same for me. I worked on the ladies department for NEXT and it was a wonderful learning and entertaining experience. As women, I guess, for me at least it does mean a lot for my clothing, style and accesories to be admired or approved by others...so if I made a serious flaw of judgement in my dress up..boy I would want to know, so that I can amend it....

I tell them.
I saw a man at work with dark trousers, he must have bumped up against something powdery and white, it was all over his butt. I am not sure what possessed me, but I started trying to brush it off with my hand. Then it kicked in that I was touching this guy's butt. Sometimes I don't think, I just start taking care of whatever is broken. It is the nurturer in me that wants to help everyone and fix things for them. Note to self: don't touch strangers, it sends the wrong message.

Hard to say, it would depend on the person and my impression of them, I guess. Sometimes it's obvious it's not a "malfunction" so much as a planned slip. As one woman told me, "we know everything we're showing".

It has happened once, and I did not say a word. I was sitting on the side of a pool casually watching a woman swim laps on her back. I suppose the pull of the water down across her chest pushed the top of her little two piece down so that one breast and nipple were completely exposed and the other a little bit. She swam another 4-5 laps like that before she was aware of it and readjusted her top.
