Can extreme fatigue cause ear disturbance?

I had horrible insomnia for 8 days. Only last night did I manage to sleep a night's worth.

However, starting yesterday, I've notice a very subtle, almost imperceptible ringing in my right ear, particularly when I get really drowsy, like now.

I've always noticed ear symptoms when I'm very tired, but this is bothering me a bit. Can fatigue be causing this or might it be more serious?
