Always worried by this, for you can have a person you love, but not truly know how they feel until something bad happens

their eyes usually. if they are severely suffering and you are a sensitive person, you can feel the sadness and misery in their aura.

You can't know with absolute certainty, but my recommendation is to pay attention to non-verbal clues. More than once I have questioned someone's response to my "How are you?" When someone says "fine", but there is no enthusiasm or their voice is cracking, most likely they are NOT fine and I do ask them if they mean that as a polite answer or as the real answer. Sometimes they end up talking about what is bothering them.

you can't know, unless they tell you. just love them unconditionally, in case they ever get the strength to ask for help.

agree, some people are so good at covering their true feelings. so well said you just need to love and protect
