Not sure about how to wake up from it. But if you know what common themes these nightmares have (This is called "Dream signs"), then you can do what's called a "reality check" any time you encounter or even think of this fear/situation like plugging your nose and trying to breathe (Sounds silly, but in a dream you can 90% of the time actually breathe through a plugged nose) and realize that it's just a dream and start doing what's called "Lucid dreaming" and do whatever you want. Look it up! It's LOADS of fun when you become accustomed to it!

It just happens and I just do. Its only on waking I realise its a bad dream. Otherwise it is real to me when I am dreaming.
