I would encourage her to tell her family herself and even go with her for moral support if she wanted me to. But I think it is her place to do the telling and not you. She told you something in confidence and if you tell her family it might be the end of your friendship.

Well, I would ask myself, if what she has done could impact the health and welfare of others, if so, yes. BUT, if the answer to that is no, as she is only hurting herself, then I have to respect it is her decision and her secret.
