    The key is to start with "With all due respect," and end with "No offense." That way you can basically say whatever you want about the person, and they can't get angry.

    Yes, when someone says "no offense" they are really just excusing themselves for saying something they shouldn't. It is like if I say "No offense, but all people with blue eyes are horrible" if you had blue eyes you would only naturally be offended even if I meant it in a way like they are horrible at checkers or something like that. When someone says "no offense" I know I am about to be offended and sometimes it offends me more that they say it. Like when someone says "no offense, but I hate when ugly people talk" they obviously just called me ugly. No, it is never a good phrase.

    If someone prefaces something with "No offense," or "Don't take this the wrong way," or, "I'm not trying to be rude," then he knows he's most likely to cause offense with whatever he's saying. So I'd probably be bracing myself for something I didn't want to hear.

    If it's something like, "No offense, but you should really do something about your weight," then it had better be coming from my doctor or a concerned family member, not some random schmo on the street. If it's something like, "Don't take this the wrong way, but you're an idiot," then why say it?
