They developed a technique to make people think differently about something, by essentially acting out a plot in their dream.
In the dream, they fell asleep and had another dream.
In this second dream, they had a third dream.
So effectively, they were working in the deepest dream, to affect the outcome in the next highest dream, which affected the outcome in the next highest dream, which affected reality.
When you dream, you can dream a story that lasts for months - even if your are only asleep for an hour. So, real life is much 'slower' than dream life.
Which explains why the van was so slow in falling - they had a long dream during the time the van fell.
They needed a 'kick' in order to wake up from one dream to get up a level, and eventually back to reality..
If they died in their dream - they died in the next reality up.
It was all clearly explained during the film.
