Betrayal without sincere remorse and amends or restitution is worse.

There are many things...(and I have experienced betrayal)
Watching my daughter go through cancer treatments....
Not having God to lean on through any of our heartaches....
Not fulfilling your purpose in life, because you can't get out of the past....
Never having the capacity to love, or be loved.....
To name a few.

Betrayal encompasses all aspects including abuse so I would have to say killing a child.

In my opion and speaking on my own behalf what's worst than betrayal too me is someone lying too me and not tell me the truth, epecially over simple stuff. I would rather be hurt by honesty and than by lies.
