it does really depend on a lot of things. ideally, yes, it would be nice to change your behavior when you see something isn't working for you--but honestly, sometimes that is much easier said than done!! for example when it comes to emotional stuff, or deeply ingrained habits.

Depends. If there were some obvious error I'd made in an otherwise correct approach the first time, then why radically alter it when all I need to do is correct the mistake? If the whole approach was flawed, that's a different story.
