Of course they would include civil marriages wouldn't they? In law there's no difference is there? In a place like Australia you'd have to count civil marriages as around 70% of all marriages are done by civil celebrants. What's the percentage in the US? I suspect a lot more people get married in churches as hell of lot more are religious nutters there.

They must---there would be no records of non-civil marriages except in the church records. You can't register a religious marriage unless it's also a civil marriage with legal marriage license, certificate, etc.

I don't know, the federal governent and the IRS doesn't count same sex couples as married even if they are in their state.
